PAH is characterized by pulmonary vasoconstriction, fibrosis, hypertrophy, and thrombosis within the small , muscular pulmonary arteries. These lead to loss of vascularity in the pulmonary circulation, and associated with these changes, is impaired production of vasodilators, such as nitric oxide and prostatcyclin that inhibit proliferation of smooth muscle, and the overexpression of vasoconstrictors such as thromboxane A2 and endothelin (ET)-1, that promote smooth muscle overgrowth.
